Frozen shoulder or adhesive capsulitis: 3 to 5 percent of the German population are either already affected by this shoulder condition or will suffer from it in the future. Two-thirds of those affected are women, and typically sufferers are between 40 and 60 years old.
The descriptive term ‘frozen shoulder’ clearly describes the problem of gradual shoulder stiffening and the affected joint, whereas the Latin term ‘adhesive capsulitis’ is typically used by medical professionals.
Frozen shoulder occurs when the tissue around the shoulder joint (glenohumeral joint), specifically the joint capsule, gradually stiffens due to thickening. The joint capsule is normally as thin as the skin of an inflated balloon; when it becomes inflamed, it is very painful and leads to increasing movement restrictions. Specific pain pressure points are uncharacteristic. In some cases, the hardened area is surgically removed to give patients suffering from unbearable pain the ability to use their arm again. The most commonly used surgical procedure is arthroscopic capsular release, in which stiff scar tissue around the joint is burned away with a heated probe. This procedure is invasive and expensive, and therefore riskier due to possible infections and complications from anesthesia. Patients who undergo this procedure often require physiotherapy afterward.
However, a study involving 500 patients in the United Kingdom found that surgery is not necessary for many of these patients. The results show that keyhole surgery or minimally invasive surgery should be reserved only for a small number of the most severe cases of shoulder stiffness, particularly in people with diabetes or other health problems who wish to avoid surgery.
What causes frozen shoulder?
In Western medicine, it is believed to be a pain transmission disorder. A malfunction of the autonomic control mechanisms in the joint leads to increased inflammation tendency and pain perception, disproportionately affecting people with diabetes mellitus, thyroid disorders, and Dupuytren’s disease. In the literature, lack of exercise and a diet with excessive animal protein are mentioned.
